וַיָּ֤קָם אָדָם֙ לִרְדָפְךָ֔ וּלְבַקֵּ֖שׁ אֶת־נַפְשֶׁ֑ךָ וְֽהָיְתָה֩ נֶ֨פֶשׁ אֲדֹנִ֜י צְרוּרָ֣ה ׀ בִּצְר֣וֹר הַחַיִּ֗ים אֵ֚ת יְהוָ֣ה אֱלֹהֶ֔יךָ וְאֵ֨ת נֶ֤פֶשׁ אֹיְבֶ֙יךָ֙ יְקַלְּעֶ֔נָּה בְּת֖וֹךְ כַּ֥ף הַקָּֽלַע׃

הגם אַ מענטש איז אױפֿגעשטאַנען זיך צו יאָגן נאָך דיר, און צו זוכן דײַן לעבן, װעט די זעל פֿון מײַן האַר זײַן אײַנגעבונדן אין דעם בונד פֿון לעבן בײַ ה׳ דײַן גאָט, און די זעל פֿון דײַנע פֿײַנט, זי װעט ער אַװעקשלײַדערן אין אַ שלײַדערלעפֿל.

May God remember the soul of my mother, my teacher,
(Mention her Hebrew name and that of her father)
daughter of who has gone to her world. because I pledge [without vowing] to donate charity for her sake. In this merit, may her soul be bound up in the bond of life, with the souls of Avraham, Yitzchak, and Yaakov, Sarah, Rivkah, Rachel, and Leah, and with the other righteous men and women who are in Gan Eden (Paradise) and let us say, Amein.
